Top Up Debt Protection Plan BEWARE

I opened a personal cheque account & credit card with FNB, through FNB consultant, Debra o Connell in May 2012. I was going through my statement last month and noticed an amount of -R136.43 for something called Top Up Debt Protection. Going back through my statements, this was debited every month (the amount varied according to my outstanding balance). Over the past year, it has totalled over R1, 000. I phoned FNB to cancel TDPP & emailed customer services 9 May requesting a reversal of all charges as I had never taken out TDPP on the card.
Their response 13 May: ... confirm that the Top Up Debt Protection Plan [TDPP] has been cancelled as requested. We are unable to reverse any fee as the plan was taken and accepted by you when the application was done on 0162012. I I did not take out this insurance. At no time did Debra o Connell explain the Top Up to me. Tired of waiting on 'feedback' from FNB.
Yesterday I tried to upgrade my gold card to a platinum card on-line. A quote was prepared for me (I did not take the Top Up Debt protection option). When I opened the new agreement, it had been automatically included! I have copies of these documents. This is wrong.
