Money lost at Old Mutual, i need IRP5

I am a pensioner who has resigned from The Department of Health in 2006 December. I got all my money due to me in 2007 June/July and transferred all the money to Old Mutual, my financial advisor was L D Mphahlele (Jarara), he resigned shortly after helping me. I got one third of the money.
I got a first letter from SARS in 2008 demanding IRP5 for the amount of R235, 751.36 the letter also said that the amount of R22, 235.68 was to be taxed from the R235, 751.36. In 2008 I was not working, I went to Old Mutual many times, and they said I must go to the Department of Health, but the Department of Health said they will not be able to help me with the IRP5 as I was not under their employ in 2008, they sent me back to Old Mutual, Old Mutual only produces IT3 instead of IRP5 each time I go to them, I went to Government Employees Pension Fund, they said i should go to Old Mutual. i went to 3 different consultants, they all say Old Mutual must give me the IRP5, If my money is at Old Mutual then why can't they give me the correct papers that I should submit (IRP5) to SARS? I am an honest citizen, I do not want to be on the wrong side of the law and end up being arrested. Please HELP!
